Form T - Application for Recovery of Gratuity

If an employer fails to pay the gratuity due under the Act, in line with the notice from the controlling authority, the employee, nominee, or legal heir to whom the gratuity is payable may apply to the controlling authority, in duplicate, on this form, to recover it.
