Form I - Application for Gratuity by an Employee
An employee eligible for gratuity under the Act, or anyone authorised in writing to act on their behalf, should apply to the employer on this form, ordinarily within 30 days of the date gratuity became payable.
If the date of superannuation or retirement is known in advance, the employee may apply to the employer up to 30 days before that date.
