Form N - Application for Direction Before the Controlling Authority

If an employer refuses to accept a nomination, accepts a claim for a lesser amount, or fails to issue either Form M or Form L within the prescribed time limit, the claimant employee, nominee, or legal heir may apply to the controlling authority for a direction, using this form, within 90 days of the cause for the application. Extra copies must be included, one for each opposite party.
