Form H - Modification of Nomination
A notice of modification of a nomination, including where a nominee predeceases the employee, must be submitted in duplicate, in the prescribed format, to the employer.
A nomination, fresh nomination, or notice of modification of nomination must be signed by the employee, or bear their thumb impression if they cannot write, in the presence of two witnesses who must also sign a declaration to that effect.
