Form 9 - Claim for Sickness/Temporary Disablement Benefit/Maternity Benefit
To claim sickness, temporary disablement, or maternity benefit, the employee must submit this claim form to the concerned branch office.
Making a false statement or misrepresentation to obtain benefit, whether for yourself or someone else, is punishable with imprisonment of up to six months, a fine of up to ₹2,000, or both.
