Form 12 - Accident Report
The employer must submit an Accident Report to the concerned branch office or ESI Dispensary within 24 hours of any accident that could result in death or disablement of the employee. Minor accidents that don't cause absence from work don't need to be reported.
ESI Corporation also allows filing the Accident Report online through the ESI Employer Portal.
