Form 20 - Claim for Maternity Benefit by a Nominee
If an insured woman dies after leaving behind a child, her nominee should submit a claim on this form on the deceased woman's account.
This form, duly filled in, must be submitted to the appropriate branch office along with a death certificate, within 30 days of the insured woman's death.
