Form 22 - Funeral Expense Claim
A nominee, or in their absence the eldest surviving member of the deceased insured person's family, should submit a claim on this form to receive the expenses incurred for the funeral.
If the claimant is a minor, the guardian should sign the claim form on the minor's behalf.
