If a nomination needs to be modified — including cases where a nominee has predeceased the employee — the employee submits a notice of modification in duplicate, in the given format, to the employer.
Any nomination, fresh nomination, or notice of modification must be signed by the employee (or, if illiterate, bear their thumb impression) in front of two witnesses, who also sign a declaration confirming this.
