Form I - Complaint to Inspector by Nominee
If the nominee of the employee has a complaint, they should make it in writing to the Inspector, on this form.
When the Inspector receives a complaint, they will examine the relevant records the employer maintains, question anyone employed there, and take down statements as needed for the enquiry. If satisfied that maternity benefit or an amount has been improperly withheld, the Inspector will direct the employer to pay it to the person claiming the payment, either immediately or within a specified period.
