Form G - Appeal to Competent Authority
An employee who wants to appeal on a maternity-related matter should make the appeal in writing, on this form, and either hand it over personally or send it by registered post to the Competent Authority.
Once the appeal is received, the Competent Authority will send a copy of the memorandum of appeal to the employer, ask for their reply, and request any documents related to the appeal by a fixed date.
