Form 6 – Record of progress of apprentice

Every employer shall maintain, for each quarter, a record in the prescribed format of the work done and studies undertaken by graduate, technician, and technician (vocational) apprentices engaged in the establishment. At the end of each quarter, the employer shall send this report to the Director of the concerned Regional Board of Apprenticeship Training.
