Form D - Statement of Employers & Employees' Contribution
The employer's contribution and the employee's contribution in respect of a year must be paid by the employer to the Karnataka Labour Welfare Board along with this statement, on or before 15 January of the following year, by cheque or crossed demand draft. The employer bears the expense of remitting the contribution.
For every employee whose name is on the register of an establishment on 31 December of a year, ₹6, ₹12, and ₹6 are payable as contribution for that year by the employee, the employer, and the State Government respectively.
