- Form 3 – Notification of due completion
- Form 4 – Eligibility certificate for the All India Trade Test, with assessment of the apprentice
- Form 5 – Index card
- Form 6 – Record of an apprentice’s progress
Applicability
- Extends to the whole of India.
- Applies only to areas or industries specifically notified by the Central Government in the Official Gazette, effective from the date mentioned in that notification.
